Privacy (Personal) - Access to Information Requests Forms (DND,VAC)

Please find attached or via our Documentation Repository the pre-filled forms to get all your personal information from DND and VAC. If you wish to receive your information on CD ROM just write this in the form.

Privacy is not simply an individual right or civil liberty; it is a vital component of the social contract between Canadians and their government. Without privacy, without protective boundaries between government and citizens, trust begins to erode.

I Strongly suggest that every Vet does an access to information. Here are the pre-filled forms and one should do Item, 1-4. If you have problems with downloading (ASKING you FOR MONEY - It out of CVA's Control) just register with Scribd, upload a file and that should solve the problem or download the attachments(s) OR DOWNLOAD THE FILE DIRECTLY AT THE END OF THE POST.

PRIVACY ACT AND INFORMATION ACT

The forms for DND, VAC, 8(M) and ESU are pre-filled you just need to complete the information requested and fax it. I’ve put in a blank form.

[4]   If you have been in trouble with the law, had external agencies which were in contact with VAC that you know of and you probably don’t know, use this form: http://www.scribd.com/doc/74437138/002-Access-to-Information-Personal-Form-350-58-e-for-VETS-VAC-8-m-Rev-2-0

[5]   If you see that there is an enormous amount of access to your CSDN [No3 above], then use this form: http://www.scribd.com/doc/74558875/002-Access-to-Information-Personal-Form-350-58-e-for-VETS-ESU-Rev-1-0

[6]   If you wish to have access to your personal information any other agency is holding on you’re, you this form: http://www.scribd.com/doc/80992966/001-Personal-Access-to-Information-Blank-Form-350-58-e-Rev-1-0 . Check for the contact info, http://www.tbs-sct.gc.ca/atip-aiprp/apps/coords/index-eng.asp#c , to send it too. I listed below the more common ones for Vets.

PPU: 055 (Pensions and Compensation), 560 (Disability Awards, Death Benefit and Detention Benefit), 030 (Treatment of a Pensioned/Awarded Condition), 550 (Rehabilitation Services and Vocational Assistance), 010 (Educational Assistance), 056 (Veterans Independence Program), 016 (Residential Care), 520 (Health Benefits Program/Public Service Health Care Plan), 020 (Health Care Programs Non-pension Related), 080 (Reviews & Appeals) and 090 (Bureau of Pensions Advocates) and All Services All Summary Assessment. Indicate for which period, from to, of time you are requesting the information. For VAc it should be the initial time you filed a pension application or you had any dealing with VAC.

For DND you should request:

PPU: 834 (Personnel Security Investigation File), 818 (Personal Information File), 836 (Unit Military Personnel Bank), 831 (Grievance File), 824 (Department of National Defence and Veterans Affairs Canada Centre for the Support of Injured and Retired Members and their Families), 817 (Canadian Forces Casualty Database), 810 (Medical Records), 811 (Dental Records), 859 (Pension File), 858 (Pay Records File), 805 (Human Resources Management Information System), 829 (DSSPM - Clothing Online). Indicate for which period, from to, of time you are requesting the information. For DND it should be the initial time you went to the recrutment centre to a current date or date of release.
